Order of Formation

Does that really matter? Does it go leader first, then top to bottom or something else?

Pre-emps first(in skill order), then leader, and whoever is next on your formation(you can read the list form for the formation).

Skills activate in this order too.

Yes, it does matter(for some) on which order you attack, so that you don't overkill/fail to kill something.

Level and innate agility also matter. The above order is correct if your monster are all the same level and same agility. A few monsters have higher innate agility than others and will attack first. Also higher level will go first if agility is the same.

your leader also gets 2x health so that can matter as well.

No it does not. The base agility of an A+ is usually 30. AA is 32 and AA+ is 34 with rare exceptions. Agility skill adds on top of that so you can figure how much agility skill is needed to be faster. In general you need an A+ with agility skill of 10 or higher to be faster than AA+ without an agility bonus.
