Name Change Empty Name Change

I looked around the forums and I couldn't find anything on this so here goes.

I was just curious about the supposed item necessary to change your name in ds. I have never actually seen one before and i was wondering if anyone has ever used one or know how it was obtained. I'm pretty sure there are none that currently exist in-game, but i'm just wondering.
